The Hammersmith& Fulham CRM Project

The Hammersmith& Fulham CRM Project is the education series delivered in collaboration with Imperial College Healthcare NHS Trust and Harrow Network Partners. The aims are to foster networking and learning around the new Primary Care Single Offer for Cardio Renal Metabolic Syndrome: taking guidance from the successful & innovative delivery model that has been rolled out in Harrow.
