Ealing Care Navigation Network Invitation

You are invited to the launch of the first “Ealing Care Navigation Network” on Monday 25 March, bringing together:

  • Care Coordinators (Ealing Community Partners)
  • Social Prescriber Link Workers (Primary Care Networks)
  • Mental Health link workers (Mental Health Integrated Neighbourhood Teams – MINT)
  • Dementia link workers (Dementia Concern)
  • Link workers (Specialist Older Adult Mental Health Services)

The aim of this forum is to build and strengthen relationships, develop community of practice, improve integration and share learning. The event will include interactive presentations, breakout sessions, opportunities to network and improve collaboration.
