The Hammersmith& Fulham CRM Project
The Hammersmith& Fulham CRM Project is the event series delivered in collaboration with Imperial College Healthcare NHS Trust and Harrow Primary Care Teams. The aims are to foster networking and learning around the new Primary Care Single Offer for Cardio Renal Metabolic Syndrome: taking guidance from the successful & innovative delivery model that has been rolled out in Harrow.

Why This matters?
Cardiovascular, renal, and metabolic conditions are deeply interconnected.
Up to 30–40% of adults are affected by obesity, hypertension, diabetes, or CKD.
Our goal: prevention rather than cure – reducing future dialysis needs and improving quality of life.
It is also an opportunity to prepare for the impending CRM contract – details TBD.

- Evidence-based templates aligned with SystmOne and Emis
- Personalised care plans focusing on diet, activity, sleep, and social factors
- Data-driven monitoring with CKD dashboards and monthly reports
Learning Events & Resources for H&F Primary Care Staff: 3 in-person Education & Networking sessions for all roles involved in CRM Care in H&F practices/PCNs 1
Session 1 was on the 11th Feb

Session 2: Preparing for the Contract was on the 18th March
Coming up Session 3: Personalised Care & Support, Wed 15th April, 13:00-16:30, ARC West London W6 9RH
Who should attend: AHPs, Health Coaches, Link Workers, HCSWs, HCAs, GPs, All Nurses & Nursing Associates would benefit from attending. Please request from your practice manager. All other roles welcome where you will be involved in delivering and advising personalised care plans and support.

- Practices that send staff to all 3 sessions will recieve a £1000 finicial incentive; for those that only attend 2 events a proprtionate incentive will be offered upon application. ↩︎
